Cantex Continuing Care Earns a McKnight's Excellence in Technology Gold Award for Using NexusConnexions to improve Care Transitions

Texas-based Cantex Continuing Care Network partnered with Nexus Health Resources® to implement a new transitional care model within its network of facilities that has improved long-term patient outcomes and reduced unnecessary hospital readmissions.

McKnight’s Long-Term Care News announced recently that Nexus Health Resources client Cantex Continuing Care Network was among a select group of winners who earned a 2018 McKnight’s Excellence in Technology Award. Cantex received the Gold award in the Transitions category for its use of NexusConnexions software to improve transitional care outcomes. 

“NexusConnexions has been a game changer for Cantex facilities,” said Lara Cline, Director of Care Coordination for Cantex. “By creating a personalized agenda for each patient and facilitating patient outreach and communication after discharge, it has helped us significantly reduce post-discharge hospital readmissions—as well as clearly demonstrate to our referral sources the specific interventions performed by Cantex staff that led to fewer readmissions.”
