We are excited to share with our healthcare partners that as of December 28, 2018, ManorCare of Northbrook, located at 3300 Milwaukee Ave. Northbrook, IL 60062, will officially become The Citadel of Northbrook. As a result, this facility will provide patient-centered care, advanced equipment and the amazing care professionals you’d expect from Citadel Healthcare. The facility offers a full continuum of care and services for short term rehabilitation, medically complex, post-surgical care, cardiac care, long-term and memory care.
