Redstone Presbyterian SeniorCare Partners with Nexus Health Resources to Improve Transitional Care Outcomes and Ensure Patient Recovery Goals Are Met

Greensburg, Pennsylvania-based Redstone Presbyterian SeniorCare is a leading provider of premier living solutions and high-quality services for older adults. The non-profit organization has partnered with Nexus Health Resources to begin using its NexusConnexions® transitional care software at the Redstone Highlands continuing care retirement community. 

 

Redstone offers distinct programs, including independent living, personal care services and in-patient rehabilitation on their Greensburg campus, Home and Community based services, through [email protected] Home Health and Hospice. They will implement the NexusConnexions software solution to more efficiently track and manage post-discharge follow-up activities and ensure quality health outcomes for patients who transition out of the Rehab Center at Redstone.

NexusConnexions transitional care software allows Redstone Highlands’ staff to better manage the day-to-day activities of a transitional care team with a tailored agenda for each patient based on individual diagnosis, risk factors, and care setting. Patient and caregiver interactions are tracked using NexusConnexions reports to compare actual and target goals and see what steps resulted in preventing readmissions.
